This form is submitted online automatically when you hit submit at the end of this form.  Please be sure to have your payment to your coach no later then the Parent Meeting on Monday October 10/17 (check our website for details).

    SVC Volunteer Bond Agreement is as follows:

    1) Tournaments and provincials require teams to provide score keepers and linesman, all parents are required to take their turn in helping with these jobs.

    2) All teams are required to host an OVA tournament as a means of fundraising for SVC to keep our registration fees reasonable. If there are not enough tournaments available to be hosted for all teams, teams will combine together to host one tournament. One parent/guardian or adult family member is responsible to volunteer at their child's hosting tournament. They can assist with set up, tear down, act as convenor for the tournament, help in food booth(running a booth is optional), score keep or line games.
    Our volunteer coaches are not responsible for assisting, even if a coach also happens to be a parent of a participant, they have fulfilled their parent obligation by coaching.
    ALL hosting teams players are required to score keep at their teams hosting tournament. If a hosting team player is unable to come and score keep, and a valid reason has been provided to their coach in a timely manner, one parent/guardian or adult family member is responsible for replacing the participant and fulfilling their duties. The participant may also be required to scorekeep at another SVC teams hosting tournament to fulfill their obligation. There are NO exceptions. All players and parents must fulfill their volunteer bond.

    I give my consent to SVC to photograph/video tape and then use, reproduce and publish said images of the above mentioned participant. I understand these images  may be used for a variety of purposes and may appear on the club website, in the local newspapers, on promotional materials or any other  media. I also understand that SVC will use the images exclusively for SVC related purposes and not for any commercial gain.  Since anyone can download an image from the Internet or make copies from printed  materials, I agree that SVC is not responsible for unauthorized use of  the images. I am aware that I am not entitled to any compensation for the use of the images.

    I / We understand that the named participant (hereafter, the “Participant”) has made an application to be enrolled in an activity conducted at, sponsored by or involved in any way with Stratford Volleyball Club (hereafter referred to as “SVC”).  The undersigned participant and / or their parents or legal guardian acknowledge that:    
    i. I / We understand that there are risks of personal injury associated with the participation in volleyball training programs, events, and activities, which can result in temporary or permanent disabilities and severe personal loss and economic damages.  
    ii. I / We understand that SVC provides no pre-enrolment medical examination and takes no responsibility for monitoring and assessing the health and physical condition of the participant.    IN CONSIDERATION OF THE ACCEPTANCE OF THE PARTICIPANT’S APPLICATION / REGISTRATION TO ENROLL IN THE SVC PROGRAM, AND WITH THE KNOWLEDGE OF THE ASSOCIATED RISK TO THE PARTICIPANT, I / WE AGREE TO THE FOLLOWING.    
    iii. I / We consent to the participant enrolling in the SVC program, and participating in the events and activities which constitute the program.  
    iv. I / We will instruct the participant to review and carefully follow all of the SVC guidelines, rules and procedures of safety and general deportment while on the SVC premises, whether or not the participant is engaged in training events or activities at the time.  
    v. I / We accept and assume full responsibility for consulting with a doctor about the SVC programs and hereby warrant, represent, and state that the participant named below is in good physical condition and that the participant has no disability, impairment, or ailment that would prevent him/her from engaging in the SVC program or any of the events or activities or that would be detrimental to his/her health, safety, comfort or physical condition.  
    vi. In the event of an emergency, I / We grant permission for medical treatment to be given at a local hospital or by trained personnel onsite.  
    vii. I / We accept and assume all risk and responsibility for accidents, illness, injury, and/or damages which may result from the Participant traveling to or from or participating in any of the events or activities associated with the SVC program, and hereby waive, release and discharge SVC, its officers, directors, employees, and agents or anyone associated with SVC from any and all liability therefore   I / WE HAVE READ THE FOREGOING AND UNDERSTAND THAT ITS TERMS INCLUDE MY / OUR CONSENT AND MY / OUR AGREEMENT TO TAKE CERTAIN ACTIONS, TO ASSUME CERTAIN RESPONSIBILITIES AND TO RELEASE SVC FROM CERTAIN LIABILITIES.  I / WE SIGN IT VOLUNTARILY WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.

    I will place the emotional and physical well-being of my child ahead of a personal desire for my child to win. I will teach my child that doing one’s best is as important as winning, so that my child will never feel defeated by the outcome of a game. I will remember that the game is for my child and that he/she plays sports for his/her enjoyment, not for mine. I will teach my child to play by the rules and to resolve conflicts or disappointments without resorting to hostility or violence or foul language. I will do my very best to make volleyball fun for my child.
    I will make my child and his/her teammates feel like winners every time by offering praise for competing fairly and trying hard. I will never ridicule or yell at my child or his/her teammates for making a mistake or losing a competition. I will help my child understand that winning AND losing are part of sports. Sports teach players to believe in themselves and to take risks; with risk comes the possibility of success and failure. Players learn important lessons from both.
    I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and referees at every game, practice, or tournament. I will remember that children learn best by example. I will set a positive example by applauding good plays/performances by both my child’s team and his/her opponent. I will refrain from making negative comments/observations about any player or coach that undermine team spirit, respect and cooperation.  
    I will support the coaches and referees working with my child, in order to encourage a positive and enjoyable experience for all. I will not question a coach or referee’s judgment or honesty in public, during a tournament, or during a practice. I will acknowledge, through my respectful behavior, the commitment to my child that the coaches make.  If I wish to discuss a concern  with a coach, I will wait until more than 24 hours has passed since the completion of a tournament and I will address the coach privately and with respect.  If I do not feel that the issue has been resolved properly, I will utilize the designated Conflict Liaison, to assist me.  
    I will demand a sport’s environment for my child that is free of drugs, tobacco, and alcohol and will refrain from their use at all events. I will support all efforts to remove verbal and physical abuse from my child’s sporting activities.   I will expect my child to treat other players, coaches, fans, and referees with respect regardless of race, sex, creed, or ability. I promise to help my child enjoy volleyball by doing whatever I can, such as being a respectful fan and learning the rules of volleyball. I will be involved and set a respectful example by ensuring my child’s attendance at practice and games on time.   I will refrain from coaching my child or other players during games and practices unless I am an official coach for his/her team.
    I understand that SVC volleyball is an elite team sport and that the position my child plays and the amount of playing time is up to the coach’s discretion for the betterment of the team.   If I should not abide by this code of conduct, I will respect the consequences of my actions as deemed appropriate by the SVC Executive. Consequences can include, but are not limited to: a verbal warning, being asked to leave a practice or tournament, suspension from future practices and/or tournaments.   By signing this document, I confirm that I have read, understand, and will abide by the SVC Parents Code of Conduct. I understand that my signature is required before my child can participate in SVC Volleyball.
